Communication Problems Contributed To Death Of Cincinnati Firefighter

A Cincinnati Fire Department report on the 2015 death of a firefighter recommends 2.3 million dollars in improvements in  communication and training.  The final report on the apartment blaze that killed Daryl Gordon shows another firefighter used a black marker to write "Do Not Enter Open Shaft" on the elevator door two minutes before Gordon went through it while searching for residents to rescue. No radio transmission communicating the warning was made.  The report lists communications failures as contributing factors in Gordon's death.
