Federal Report On Death Of Cincy Firefighter Backs Up Department's Conclusions

An investigation by the National Institute for Occupational Safety and Health into a fatal Cincinnati fire says a series of mistakes by firefighters led to poor visibility for the one who died when he fell five floors down an elevator shaft. The report says a delay in getting water on the fire contributed to smoky conditions. It says visibility was 5 feet or less when Daryl Gordon opened the elevator shaft door. The investigation largely agrees with one released by the Cincinnati Fire Department that concludes Gordon died last year when he walked through an elevator door that should have been locked. It also highlights mistakes by fire crews leading up to the fatal fall.
