The Ohio Department of Rehabilitation and Corrections has disciplined four employees for lapses that helped the Chardon High School shooter and two other inmates escape from the Allen Correctional Institution near Lima last September.

Defense attorneys for the teen charged with killing three Chardon High School students last February have entered a not guilty by reason of insanity plea on their client's behalf.
